We're here to help!


We offer Support at Home Packages, Commonwealth Home Support Services and can also provide you with private or fee for service options.  Our aged care services are aimed at keeping you well, independent and living your best life.  We can tailor a solution that best suits your needs.  

Personal Care 

Support workers can assist you with a range of personal care supports including, hygiene assistance, assistance with aids and appliances or assistance with your continence care needs. 




Medical Appointments

Transportation assistance service specifically designed to help elderly individuals reach their medical facilities for appointments.




Nursing Services

Our Nursing team are able to assist you with your clinical requirements including wound care, catheter management and provide support and education around your health needs.   

Aquafit

Gentle water-based exercise designed to increase physical fitness for the aged and people who have a disability.


Support at Home Program

A Support at Home Program is a government-funded program that allows older people to receive care services in their own home, so they can live independently for as long as possible.

Meal Preparation

We specialise in providing food preparation assistance specifically tailored for aged care. We understand that seniors may face unique challenges when it comes to meal planning and cooking, and our goal is to make their food preparation experience safe, convenient, and enjoyable. 

Reablement 

We offer Aquafit which has huge benefits to individual’s confidence, functional ability, mobility, independence, and wellbeing.

CHSP

The CHSP (Community Home Support Program) provides entry-level support for older people who need some help to stay at home.



Day Respite or Overnight Care.

Our programs offer a supportive environment where participants can take a break from their daily routines and receive personalised care. We offer engaging activities, round-the-clock assistance, and a comfortable setting to enhance their well-being.


Overnight/ Day Respite

Our day respite center offers a range of programs, activities and community excursions for people over 65 years of age.

Fee for Service 

It is an individual form of payment for healthcare services to provide you with support services when needed.




Ground Control, Domestic Gardening Assistance

Our Ground control team pride themselves on providing you a domestic gardening service. Including lawn mowing, pruning, pressure washing of paths as well as completing other essential gardening activities aimed at keeping your outdoor areas safe.

Introducing Our New Health & Wellbeing Program!

Our land-based exercise program has kicked off, every Wednesday. This program is an exciting extension of our beloved Aquafit program and will be generously supported by the same funding.

Join us for gentle yet effective exercises designed to enhance strength and balance, suitable for all fitness levels.

For those new to Aquafit, an initial assessment by our friendly Exercise Physiologist is required before diving in. We can't wait to see you there!

Quality Advisory Committee

At Gladstone Community Linking Agency, we are committed to providing safe, respectful and high quality aged care services.

Our Quality Advisory Committee helps us do this.

The Committee is a group that oversees the quality and safety of our aged care services. They regularly review how we are performing and look at ways we can improve.

The Committee:

• Reviews feedback, compliments and complaints
• Monitors incidents and how they are managed
• Looks at safety and risk issues
• Reviews audit results and compliance requirements
• Identifies areas for improvement
• Ensures customer wellbeing, dignity and rights are upheld

This Committee helps make sure we are meeting our responsibilities under the Aged Care Quality Standards and continuously improving the way we support our customers.

Our Customer Advisory Committee is a group that brings together customers, representatives and members of our team to share feedback, ideas and lived experience. The purpose of the Committee is to make sure customer voices directly influence how we improve and deliver our services.

The Committee:

• Provides feedback on services and customer experience
• Shares ideas to improve supports and communication
• Reviews proposed changes that may impact customers
• Helps shape new programs and service improvements
• Ensures customer perspectives are heard at leadership level
